Hi all, Even though I’ve switched roles with my partner again, I decided to wear the device for a day or two in intervals so I don’t lose that sense of being used. With a major exam coming up next month where I’ll be studying 12 hours daily, it seemed like a good idea since I’ll mostly be sitting and reading anyway. Also, my doctor prescribed me Adderall for the first time to help maintain focus over the next few weeks. I began taking it earlier in the month without issues, so I couldn’t see why it would affect my penis while caged; after all, the drug’s vasoconstrictive properties are minimal unless the dosage is very high.

I woke up, locked myself in, took my medication, and headed to study. As the day progressed, I noticed a dull ache developing in one testicle and the side of my scrotum, which I initially blamed on being outside the device for too long. Even after taking ibuprofen, it kept getting worse until the pain radiated up into my abdomen, prompting me to remove it right away. The discomfort went down but was quite sharp during that time. I looked through some medical resources (like Uptodate and PubMed) along with ADHD discussion boards and discovered that another similar ADHD medication often causes testicular pain; several Adderall users have reported the same issue, even though it isn’t currently listed as a side effect.

This was quite interesting to me. I read further into a urologist’s blog post explaining how constant norepinephrine activation over time, combined with other neurotransmitter imbalances, can trigger testicular pain due to an excess of PGE2, an inflammatory mediator that increases pain sensitivity. (The part about the vas deferens’ elasticity was where I got lost.)

So here is a heads-up for anyone else who is locked up and considering trying a stimulant like Adderall: keep an eye out for this specific side effect. If you take it off when the symptoms first appear, there shouldn’t be any long-term damage—I’m feeling completely fine today now, and everything seems to be working well down there—but sustained or chronic inflammation of testicular tissues can lead to serious complications.
