I sometimes have to switch the standard-sized rings for the smaller ones so everything stays put. Since my wife and I are hoping for more children, I worry that reduced blood flow to the testicles could impact our fertility.

You're worrying unnecessarily. There was a couple on this forum who had a baby about a year ago. He spent most of his time locked up, but she kept him well under control and still got pregnant.
