PA5000 discussion

I’m picking up a PA in March and thinking about getting the PA-5000 until I can afford a Lori device (eyeing model #7). How does the 5000 function without a ring encircling the testicles? It seems like it might pull on both the shaft and piercing when soft, but if you get erect, it looks almost usable as a cock sleeve for masturbation. Probably an obvious question, just curious.
